Family Reunification Practitioner
The Reunification Practitioner (Practitioner) works closely with families and the Department of Child Protection (DCP) to create a reunification plan that makes it safe for children in out of home care to return to the full-time care of their parents. The Practitioner works within multi-agency teams to collaboratively assess risk and put safety measures in place. Practitioner provides intensive and therapeutic support, advocacy, and information. Engagement takes place in a variety of settings including phone calls, face to face meetings in an outreach setting, this may include home visits.

The successful applicant will possess skills, experience, and knowledge in working with Aboriginal people and their communities. The Practitioner will operate within a culturally therapeutic, trauma informed framework. The applicant would have demonstrated knowledge of the child protection statutory system, complex case management and working with Aboriginal families.
Kinship Care Practitioner
The KWY Kinship Care practitioner can conduct home/outreach visits, relevant assessments, facilitate training, write reports as required, attend multidisciplinary meetings, keep accurate and timely case notes and communications. and advocate for Kinship Carers in a variety of settings. The practitioner aims for children and young people to feel safe and supported. The practitioner achieves this by supporting carers to connect children to their culture, kin, and identity. The practitioner works with Government agencies and external service providers to support Kinship Carers to meet the children’s developmental needs.

The role can entail intrastate case management and travel to the Far North, Eyre and Yorke Peninsula, Limestone Coast.

The position is located at the KWY Head Office, Parkside, Adelaide.

The successful applicant will possess 2 years of skills, experience, and knowledge working in kinship or foster care programs or equivalent experience. You can demonstrate safe cultural practices when collaborating with Aboriginal people and community. Your approach is strength focused, through a cultural lens listening to the voice of carers and children.
Practitioner Your LINC Mental Health
Your LINC (Lived Identity Navigate Connect) is a multi-agency, two-tier mental health outreach service delivered in partnerships with Centacare and KWY Aboriginal Corporation. Our service offers holistic, flexible and culturally responsive phyco-social support for individuals experiencing mental health challenges and distress across Metropolitan Adelaide and some peri urban post codes.
KWY provides non-acute psychosocial support through the provision of case management to support people to build independence connect with community and improve quality of life over a period of 4 – 12 months.

The role involves providing the direct support and case management to Aboriginal and Torres Strait Islander people who provided non-acute psychosocial services and intensive crisis support services. Mental Health Practitioners are responsible for guiding the delivery of the Your LINC service for KWY across Metropolitan Adelaide and will report directly to Program Coordinator.

You have worked in psychosocial and social and emotional wellbeing for Aboriginal and Torres Strait Islander people and are able to work with various stakeholders internal and external, within a multi-agency team. You have an understanding of the barriers experienced by Aboriginal and Torres Strait Islander people in accessing mainstream Mental Health services and are familiar with working within a Social and Emotional Wellbeing Framework.
Practitioner
The KWY Practitioner is a highly experienced and competent worker, who delivers dedicated support and services to individuals, children and families. The Practitioner is responsible for contributing to improvements in the quality, safety and community needs-driven distribution of services that meet the needs of Aboriginal children, young people, and their families. Practitioners work to assess risk and put safety measures in place while providing intensive and therapeutic support, advocacy, and information. Engagement takes place in a variety of settings including phone calls, home visits and at community services. We are also seeking Practitioners who have skills working directly with children.

The successful applicant will possess skills, experience, and knowledge in working with Aboriginal people to address child protection and domestic and family violence concerns and for the case of working directly with children The Practitioner will have an understanding of children’s work, all Practitioners need to operate within a therapeutic, trauma-informed and culturally responsive framework. The applicant would have demonstrated knowledge of complex case management and working with Aboriginal and Torres Strait Islander families.  
NDIS Access Practitioner
Practitioner supports Aboriginal and Torres Strait Islander people to gain access to the NDIS. The Practitioner is responsible for writing the clients NDIS application and gathering the necessary documentation of evidence to demonstrate the client’s eligibility for NDIS. Clients may have complex needs and multiple barriers to engagement including mental health issues, homelessness, domestic violence, child protection concerns and cognitive disability. We are seeking a compassionate and resilient Practitioner for permanent (or part time 0.8 negotiable) opportunity to work in the Adelaide metropolitan area. The Practitioner delivers a culturally appropriate, person-centered approach to people living with a disability. The Practitioner will have great skills in advocacy, arranging assessments, navigation of the NDIS, and strong skills in report writing.

The successful applicants will possess skills, experience, and knowledge in working in disability sector.
You will need to have:

Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or another related allied health field.

Expertise in working outreach with Aboriginal people with disabilities, complex needs and interacting with families and carers.

Demonstrated understanding of the NDIS, Community Services networks, and agencies with ability to build strong relationships with service providers, clinical teams, and key stakeholders.

Must hold a current South Australian drivers’ license, be willing to drive a work vehicle.

Must hold or be willing to obtain and keep current, Child Safe Environments Training (Through Their Eyes).

Must hold a current or be able to obtain the following: Australian Working Rights, Department of Human Services (DHS) Working with Children Check, Working with Vulnerable Person’s Check and National Police Check.
