What We Offer

KWY’s Post Care Support Service provides support for Aboriginal young people who are transitioning from out-of-home care and/or Guardianship of the Chief Executive of the DCP into independent living. This service is run in partnership with Relationships Australia South Australia and Anglicare-SA. 

 

Support can include (but not limited to): 

 

• Linking in with relevant services through warm referrals. 

• Providing support letters and advocacy in areas such as housing, education and employment. 

• Support to develop and expand on independent living skills. 

• Together developing support plans with relevant services as well as those important people in your life.

Eligibility

Aboriginal young people aged 17-25 years who are currently or were previously in out-of-home care for six months or more.  ​
